I am considered an ‘out of network’ provider, so you will need to check and verify that your health plan offers out-of-network reimbursement. If so, I am happy to offer you a receipt for your treatment . which you may submit to your insurance company for reimbursement. If you are unsure, I recommend calling your insurance provider to verify that they accept out of network Acupuncture.

How to Submit an Insurance Claim

Following Treatment, you will receive a Super Bill. A Super Bill is a receipt which includes essential details about your visit, including the services received, provider information, and payment details. You can then submit this document to your insurance company for reimbursement. Make sure to call your insurance company and verify the mailing address or fax number for submitting your super bill, and ask whether any other information is needed.
